TO+ Views
TO+ visualises information real time recorded through the app, gives a clear outline of each client's or group of client's progress and allows for systematic knowledge sharing in order to initiate appropriate actions.
The views are customisable and can be branded to your organisation.
The standard view is a triage dashboard that shows a group of clients and their triage, tool registrations and development over time.
More elaborate views can show extended details and handle due tasks, medicine, messages and focus areas.
TO+ detailed view shows each individual registration such as test results and give recommendations for next step or warnings if action is needed.
Client insight gives an overview of all tool registrations and development for a specific client.
Statistics
TO+ comes with a statistics module to give insight into registrations and development of triage as well as usage. Giving you valuable insight into health status both on a group and individual level.
The TO+ App
At the core of TO+ is the TO+ app with its intuitive userinterface it allows for quick registration of a clients condition with a few clicks.
The TO+ app consists of various tools for early detection of health issues and functional decline.
We have a variety of tools available and develop new tools based on a continuous dialogue with our clients about their needs. This includes tools or views designed to support specific practices.
The TO+ app displays a client or a group of clients
sorted in regard to their triage and recent changes.
Automatic calculations and pre-recorded recommendations save time and enable the worker to make the right call in each situation.

The logbook
The logbook gathers all the information registered about a client in one place. This gives an overview of which
registrations are made in the individual tools by whom.
TOBS
TOBS provides an opportunity to collect vital parameters in order to detect any beginning illness and take appropriate action. On the basis of the registrations, a recommendation and a proposal for triage are presented.
The Wheel
The wheel has six focus areas and enables the registration of specific observations and reflection upon these.
The Change Chart
The change chart is used to set and describe the client's habitual state and monitor changes. The change chart reinforces preventive efforts by focusing on changes in the client's condition.
In addition to the displayed tools, you can choose among a number of extra tools designed to support specific needs and practices.
We furthermore develop new tools based on need by our customers and clients.
How does it work?
Observations are registred with a few clicks on the app. The data is automatically saved to the central database and the progress in the client’s condition can be monitored on the dashboard.
TO+ saves precious time by calculating scores and offering recommendations. Everyone involved in the care have access to all registrations, without having to resort to other journal systems.
Information is automatically shared among the different professionals involved in the care of each client. Optimised inter-professional collaboration allows for swift and adequate response.

The clients themselves can follow their health status on the same platform and add data through the tools.

With TO+ you get more than just an IT system. We assume responsibility for the entire implementation process, focusing on customisation and local implementation.

As part of any implementation we design appropriate workflow together with our clients to ensure maximum adaptability and simulate expected work practice before implementation to address any issues that could potential arise.
